If you have already booked the appointment please complete the Registration Form below.  Once completed, you will be sent an email which you have to click the link to sign.  You and we will then receive a PDF copy of the form and you have registered with us as a patient at that point. Please make sure before signing to fully read the third page as this covers a number of legal points.  If you have any questions please email us at marc@penicuikpodiatrist.co.uk with any questions.  If you are not able to complete online then come into the clinic about ten minutes before your appointment to complete.  Note that we will not be able to treat you without this form being fully completed.
