We will phone you the day before the booking to give you the final time.

If this is your first appointment you will have to complete a registration form.  You can do this online there is a form at the bottom of this page.  The form must be signed by a holder of power of attorney if appropriate.

Payment is by card only.  If being paid for by a relative you can phone the clinic and pay over the phone.
